There are four (4) graduate forms that need to be completed as part of graduation requirements.
Form 1: Proposed Graduate Course Plan
Form 2: Part A: Thesis/Project Committee; Part B: Topic Approval and IRB Verification
Form 3: Preliminary Graduation Notice
Form 4: Application for Graduate Degree
