Release Form for Designated Student Classwork to be Placed on Reserve at the Library

The Release Form is available in four different formats for your convenience:
                PDF (can be filled out online and then printed)
                Microsoft Word 95 (must be printed first, then filled in)
                Microsoft Word 97 (must be printed first, then filled in)

To use the online PDF format, your computer must have Adobe Acrobat Reader.  This program is available free-of-charge at the following URL:  http://www.adobe.com/prodindex/acrobat/readstep.html

1. The online PDF form must be filled in and then printed.  This form requires a signature so it cannot be submitted electronically.

2. Tab between questions.  Complete all appropriate information.

3. Fit the answers into the space provided.  Only what you see on the screen will print.

4. Print the completed form and sign it.  Submit the signed form to Jenny Jensen, Library.
