In addition to other forms of advertising, the Office of Student Activities encourages you to utilize our facebook page to post meeting and event information for your organization. We have hundreds of students who follow our page for information on everything going on around campus, and we are always looking for more organizations to contribute!

Advertise your event with this form.The Office of Student Activities has a large, monthly wall calendar and online events calendar available to all student organizations and campus departments to advertise upcoming meetings, special events, and recruitment activities.

Storage cabinets are available for recognized student organizations in the Activities Resource Center. Applications will be accepted starting the beginning of April for the next academic year.

Funding Requests & Reimbursement:
- This form is for student organizations who want to start an agency account.

This form is used when people have lost the receipts or did not get a receipt for the purchases they made for a student organization and need to be reimbursed from a student activity account.
**These require a notary public to sign and stamp the form. A notary public is available to student organizations at Affinity Plus in the CMU.

This form is for student organizations who wish to use money allocated from the previous fiscal year in the current fiscal year.
** This applies to SABC funding.

Used to deposit money into student organization’s activity or agency accounts

Students asking businesses for donations should always offer this form.This form gives businesses an option to receive tax credit for their gift.

This form is used to reimburse people from a student agency account for expenses they’ve paid out of their own pocket for the student organization’s activities.
Student Technology Fee Funds Request
Student Travel Funding Application
- This form is for
requesting travel funds not previously allocated to an organization’s
base budget, post season travel funds or for individual MSUM students
requesting travel funding assistance.
**Allowable expenses include: lodging, transportation, registration fees, event parking, supplies, and meals for students.
Supplemental Funding Application
- This form is given to students or student organizations that need additional funds for a variety of reasons.
**This form is not used for travel funds (see Student Travel Funding Application).

This form is used to reimburse people from a student activity account for expenses they’ve paid out of their own pocket for the student organization’s activities. **Receipts for the purchases must be attached to this form.

Student organizations using state vehicles for any reason, including but not limited to errands, travel or for event purposes, must have each driver read the vehicle safety program information, fill out the online Vehicle Use Agreement form and be familiar with motor pool policies. Driving records will be checked with the Vehicle Use Agreement form information. Each driver must have an acceptable or conditional driving record according to the Department of Motor Vehicle check to use a state vehicle.
